We are interested in hearing from you about current school/daycare policies and practices in Quebec to protect children who have asthma and/or food and other allergies which can be life-threatening. We would also like to know how you feel about your child’s situation, for example, how supportive his/her school or daycare community is and how well prepared staff are to react in an emergency.

The survey takes about 10 minutes to complete. (Please complete ONLY if your child attends a school or daycare in the province of Quebec.)

Nous sommes intéressés à connaître votre opinion concernant les pratiques et procédures en place au Québec afin de protéger les enfants souffrant d’asthme et/ou d’allergies pouvant causer la mort. Nous aimerions également savoir comment vous vous sentez face à la situation que vit votre enfant. Par exemple, estimez-vous que son école ou service de garde (CPE, garderie et service de garde en milieu scolaire) vous offre un support adéquat et que le personnel est suffisamment préparé à réagir en cas d’urgence?

Ce questionnaire prend une dizaine de minutes à compléter. (SVP compléter ce sondage si et seulement si votre enfant fréquente un service de garde ou une école au Québec).
